Responsibilities

• Provide social services to residents and their families under supervision.
• Meet new residents on admission; complete Initial Psychosocial Assessments.
• Coordinate with medical and care teams; arrange community resources and support services.
• Assist with Medicaid, Medicare, SSI documentation and eligibility.
• Support residents and families during loss and transitions.
