Job summary

St. Francis Center for Rehabilitation & Healthcare, a 273-bed facility, seeks a Director of Social Services to lead the Social Services team and coordinate resident care based on emotional and psychosocial needs.

The role develops programs, performs psychosocial assessments, coordinates transitions and discharges, and advocates for residents' best interests, collaborating with administration and clinical staff.

Qualifications

• Bachelor's Degree required.
• Long Term Care experience required.

Responsibilities

• Assists with resident and family transition into the facility
• Completes psychosocial assessments identifying social, emotional, and psychological needs and strengths
• Identifies medical-related social needs and provides appropriate services
• Maintains resident social information and files on admission
• Plans, organizes, and implements Social Service programs
• Evaluates effectiveness of Social Service programs in collaboration with staff
• Coordinates discharges and advocate for residents' best interests
• Conducts resident interviews and ensures MDS sections and Care Area Assessments are completed
