Job summary

AA Healthcare Management seeks a detail-oriented, empathetic Resident Services Coordinator to manage onboarding, administrative operations, and the care transition pipeline for our residents. You will serve as the primary liaison between families and our internal clinical team.

You will collaborate with discharge planners, clinicians, and families, ensuring accurate data and smooth transitions in a fast-paced, tech-enabled environment.

Qualifications

• 1–3 years of experience in a long-term care, assisted living, or skilled nursing facility (SNF) setting.
• Experience using modern healthcare tech, CRMs, or EHRs. Direct experience with ExaCare is highly preferred, but training will be provided for candidates with strong digital literacy.
• Exceptional verbal and written communication skills for interacting with hospital discharge planners, families, and clinicians.
• Ability to multitask in a fast-paced environment while maintaining meticulous data accuracy.
