Job summary

Avina on 32nd is seeking a detail-oriented, empathetic, and tech-savvy Resident Services Coordinator to manage onboarding, administrative operations, and the care transition pipeline for our residents. You will serve as the primary liaison between families and our internal clinical team.

The role requires 1–3 years in a long-term care setting, familiarity with healthcare tech and EHRs, and strong communication skills to coordinate with hospital discharge planners and clinicians.

Qualifications

• Experience with modern healthcare tech, CRMs, or EHRs.
• Direct experience with ExaCare is highly preferred, but training will be provided for candidates with strong digital literacy.
• Exceptional verbal and written communication skills for interacting with hospital discharge planners, families, and clinicians.
• Ability to multitask in a fast-paced environment while maintaining meticulous data accuracy.

Resident Services Coordinator Qualifications
