Job summary

Qualifications

• Active, unrestricted clinical license in state of hire (RN/PT/OT/SLP).
• Minimum 5 years of clinical experience.
• Must reside within 30 miles of the job location with reliable transportation.

Responsibilities

• Engage with patients and families weekly for discharge goals and barriers.
• Collaborate with health care teams in interdisciplinary meetings.
• Review discharge plans with providers and families.
• Perform SNF assessments using CMS criteria upon admission and during stay.
• Maintain timely, accurate documentation and protect PHI.
