Job summary

Victorian Post Acute in San Francisco, CA, is seeking a Social Services Assistant to support residents and families with their social, emotional, and psychosocial needs during their healthcare journey. This full-time role offers meaningful relationship-building and a chance to impact quality of life.

You’ll collaborate with the care team, assist with admissions and discharges, and help connect residents to community resources. Prior healthcare exposure is preferred; PCC experience is a plus.

Qualifications

• High school diploma or equivalent.
• Previous experience in healthcare, skilled nursing, senior living, or social services preferred.
• PointClickCare (PCC) experience is a plus, but not required.
• Excellent communication and interpersonal skills.
• Strong organizational and documentation skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Ability to work effectively with residents, families, staff, and community resources.
• Empathetic, patient, professional, and resident-focused approach.

Responsibilities

• Build positive, meaningful relationships with residents and their families.
• Provide compassionate support for residents’ social, emotional, and psychosocial needs.
• Assist with care planning and help identify goals that promote independence, dignity, and quality of life.
• Provide emotional support to residents and families experiencing illness, disability, adjustment, grief, or loss.
• Assist with admissions, transfers, and discharges.
• Participate in discharge planning and help connect residents with appropriate community resources.
• Interview residents, guardians, and sponsors to obtain required admission information.
• Prepare and organize admission packets, including resident rights, privacy notices, and admission agreements.
• Maintain accurate resident records and identification information.
• Maintain accurate bed availability records.
• Collaborate with nursing, rehabilitation, administration, and other interdisciplinary team members.
• Maintain resident confidentiality and follow facility policies and applicable privacy requirements.
