Job summary

Ohio Living is seeking a Social Services Designee to plan, organize, implement and evaluate social services for residents within our life plan community in Ohio. You’ll be the liaison between residents, families and community resources to address emotional and psychosocial needs.

Responsibilities include admissions planning, care coordination, counseling, and collaboration with healthcare teams. A high school diploma is required; an associate degree is preferred.

Qualifications

• Contains social history on admission and reviews with supervisor.
• Experience in health care or human services environment preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

Responsibilities

• Assists in planning, organizing, implementing, and evaluating social services for residents and patients.
• Coordinates social services activities with other departments to ensure daily programming proceeds without interruption.
• Complete social history and care plans, maintain EMR documentation, and protect confidentiality.

Education

High school diploma or equivalent

Associate degree in a related field

Ohio Board of Nursing Certified Community Health Worker training or willingness to complete
