Job summary

The successful candidate will conduct psychosocial assessments, assist with the admission and discharge processes, document needs, and provide emotional support.

Qualifications

• Experience in long-term care, social services, case management, or senior care preferred.
• Ability to work with diverse resident populations and their families.
• Strong communication, empathy, and interpersonal skills.
• Organized, dependable, and able to maintain confidentiality.
• Knowledge of state and federal long‑term care regulations a plus.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Assist with the psychosocial assessment of new residents.
• Support the admission process, ensuring residents and families feel welcomed and informed.
• Participate in care plan meetings and document resident needs and preferences.
• Provide emotional support, crisis intervention, and problem‑solving assistance.
• Communicate with residents, families, and interdisciplinary team members.
• Assist with discharge planning and coordination of services.
• Maintain accurate documentation in accordance with state and federal regulations.
• Advocate for residents’ rights and help ensure a positive care experience.
• Assist residents with completing forms, applications, and service coordination.
• Participate in resident and family education regarding available resources.
