Job summary

Western Hills Retirement Village, a Caring Place Healthcare Group Facility in Cincinnati, is seeking a Social Services Coordinator to support residents and families with psychosocial, emotional, and environmental needs; join a team that values empathy, communication, and person-centered care.

In this role you will complete assessments and care plans, advocate for residents’ rights, coordinate with interdisciplinary teams, ensure HIPAA privacy, and help sustain a calm, respectful environment

Qualifications

• Bachelor’s degree in Social Work or related field from an accredited college or university.
• Current Ohio Social Worker license.
• Minimum of 2 years of healthcare experience preferred.
• Excellent communication and customer service abilities.
• Ability to maintain accurate and timely documentation.
• Ability to read, write, and speak English.

Responsibilities

• Provide social services support to residents, addressing psychosocial, emotional, and environmental needs.
• Communicate with residents and families to resolve concerns, provide guidance, and ensure needs are met.
• Complete and maintain comprehensive assessments, including social histories, care plans, and progress notes.
• Advocate for residents’ rights, ensuring dignity, privacy, and respect at all times.
• Collaborate with interdisciplinary teams, including staff, consultants, and healthcare professionals.
• Maintain strict confidentiality of resident information in accordance with HIPAA regulations.
• Support a calm, caring, and respectful environment for residents and staff.
• Participate in care planning and contribute to individualized resident care strategies.
• Attend required trainings, in-services, and meetings.
