Job summary

Western Hills Retirement Village in Cincinnati, OH, is seeking a Social Services Coordinator to enhance resident well-being through psychosocial support, assessments, and care planning. This is a full-time, on-site role with a competitive salary and company-paid health insurance.

The ideal candidate has a Bachelor's in Social Work, an Ohio Social Worker license, and at least 2 years in a healthcare setting, with strong communication, documentation, and teamwork skills.

Qualifications

• Bachelor's degree in Social Work or related field.
• Current Social Worker license in the State of Ohio.
• Minimum of 2 years of experience in a healthcare setting preferred.

Responsibilities

• Provide social services support to residents, addressing psychosocial, emotional, and environmental needs.
• Complete and maintain comprehensive assessments, including social histories, care plans, and progress notes.
• Advocate for residents’ rights, ensuring dignity, privacy, and respect at all times.
• Collaborate with interdisciplinary teams, including staff, consultants, and healthcare professionals.
• Maintain strict confidentiality of resident information in accordance with HIPAA regulations.
• Support a calm, caring, and respectful environment for residents and staff.
• Participate in care planning and contribute to individualized resident care strategies.
• Attend required trainings, in-services, and meetings.
