Job summary

NHS Management, LLC is seeking a Social Services Assistant to act as a liaison for residents, plan and develop the social services program, and ensure compliant care. The role involves coordinating with residents’ families, staff, and community agencies to meet medical, emotional, and social needs.

Essential duties include maintaining resident rights reviews, completing forms, and supporting the admission process while upholding HIPAA and regulatory requirements.

Qualifications

• High School graduate or equivalency.

• Must have patience, tact, enthusiasm, and positive attitude toward the elderly and disabled.
• Must be at least 18 years of age.
• Must be able to speak, read, write, and understand English.
• Must be capable of performing the Essential Job Functions.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, supervising and directing the social services programs.
• Participate in ongoing quality assurance program.
• Maintain resident right reviews and advance directives.
• Complete necessary forms, reports and progress notes.
• Participate in community planning and discharge planning.
• Assist with admissions and coordinate with families and staff.
