Job summary

Apploi is seeking a Social Services Assistant to support the implementation and evaluation of social services programs within a skilled nursing facility in Los Angeles. The role emphasizes resident dignity, clear communication, and collaboration with the care team to meet resident needs under federal and state standards.

Responsibilities include assisting the Director of Social Services, arranging transportation and appointments, providing residents and families with Medicare/Medicaid

Qualifications

• High School diploma or GED is required or preferred.
• Social Services designee preferred.
• Previous experience in skilled nursing is preferred.
• Must be able to read, write, speak, and understand English.

Responsibilities

• Assisting Director of Social Services.
• Making phone calls to set up transportation or doctor appointments for residents.
• Providing residents and family Medicare/Medicaid and financial information when requested.
• Maintaining accurate social service progress notes.
• Updating the social services directory.
• Obtaining information from community social health and welfare agencies to meet residents' needs.
• Planning and organizing time and duties to efficiently maintain caseload.
• Reporting occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately.
• Protecting resident files and data with confidentiality per HIPAA standards.
