Job summary

SAK Management seeks a Social Services Aide to support planning, organizing, and directing social service programs for facility residents. The role ensures medically related emotional and social needs are addressed on an individual basis while safeguarding residents’ health, safety, and welfare.

Qualifications

• Must possess a high school diploma; bachelor’s degree in social work preferred but not required.
• Experience in social services or resident support preferred.

Responsibilities

• Greets new residents in the facility and orients them to the building and staff.
• Maintains appropriate departmental documentation and completes social service admission notes.
• Assesses all new residents upon admission and completes social history and social assessment.
• Writes quarterly and annual updates of Social Assessment and Social History.
• Records significant events in residents' lives and social service contacts.
• Informs residents and families about funeral pre-arrangements and follows up as needed.
• Completes care plans and follows through with goals.
• Interviews residents and families as necessary in private settings.
• Maintains contact with residents and families to identify changing needs.
• Communicates with residents with communication problems and relays needs to family and agencies.
• Coordinates discharge planning and maintains notes for appropriate residents.

Education

High school diploma required; Bachelor's degree in social work preferred but not required
