Job summary

PACS Group in California is seeking a dedicated Case Manager to coordinate comprehensive plans of care for residents. You will work with residents, families, and healthcare providers to develop cost-conscious, outcome-focused care plans and ensure smooth admissions and discharges.

Responsibilities include utilization review, accurate documentation, and collaboration with care teams and payers. Strong communication and organization are essential in our fast-paced environment.

Qualifications

• Bachelor’s degree preferred; healthcare experience beneficial.
• Experience in healthcare, case management, or social services preferred.
• CCM certification a plus.
• Knowledge of Medicare/Medicaid programs and benefits.
• Ability to organize, prioritize, and manage multiple tasks in a fast-paced environment.

Responsibilities

• Coordinate care with residents, families, providers, and payers.
• Oversee admissions and discharge activities.
• Complete utilization review for appropriate, timely, cost-effective care.
• Maintain accurate charting and timely documentation.
• Communicate Plans of Care and treatment protocols to team and residents.
