Job summary

PACS Group in Arizona seeks an experienced RN/Case Manager to collaborate with residents, families and healthcare providers to develop and implement care plans that balance extraordinary patient care with cost efficiency.

You will coordinate admissions, discharges and referrals, document thoroughly, perform utilization reviews, and ensure timely communication of plans of care during meetings and with the care team.

Qualifications

• Bachelor's Degree in Nursing or Social Work required or preferred.
• RN license preferred; LVN/LPN acceptable.
• Minimum 2 years clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs.
• PCC knowledge preferred.
• CCM certification is a plus.

Responsibilities

• Communicate plans of care and treatment protocols with residents and families.
• Attend and participate in morning meetings to facilitate team communication.
• Answer residents' questions about care, illness progression and treatment.
• Monitor and adjust residents' statuses based on changing needs.
• Organize and prioritize daily work for resident care and documentation.
• Perform utilization review to ensure cost-effective care.
• Coordinate care with residents, providers, facilities, and payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Maintain professional, respectful interactions with residents, families, staff and partners.
• Complete paperwork accurately and timely.
