Job summary

PACE Southeast Michigan seeks a Community Living Associate (CLA) to provide supportive services to residents in the community living setting, day health center, and during transport. You will follow individual care plans, assist with personal care, meals, activities, and coordination of care under supervision.

The role requires a high school diploma or GED, valid Michigan driver’s license, CPR, and a year of experience with frail or elderly populations.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• One year experience with a frail or elderly population preferred.
• Completion of a basic nursing assistant training program outside of PACE SEMI.
• Medically cleared for communicable diseases and up-to-date immunizations.
• Ability to establish and maintain interpersonal relationships and work with diverse backgrounds.
• Ability to problem solve and interpret instructions.
• Ability to organize and maintain flexible scheduling.

Responsibilities

• Assist participants with daily living activities per the care plan (2-person assist max).
• Provide 1:1 interaction in day health center and community living setting as outlined.
• Assist with meals, nutrition, toileting, and incontinence management.
• Help with housekeeping, groceries, laundry, and other chores.
• Coordinate care with the DHC and caregiver team as per the plan.
• Report deviations, issues, and emergencies to supervisor.
• Maintain accurate, timely documentation of services provided.
