PACE SEMI seeks a Community Living Associate to support residents in community living settings, during transport, appointments, and day health activities under supervision of the manager, RN, or Lead CLA. You will follow each participant's care plan and document care provided.

Requirements include a high school diploma or GED, a valid Michigan driver's license, CPR certification, and at least one year working with frail or elderly populations in long-term care, hospital, home care, or PACE

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• One year experience with a frail or elderly population.
• Experience in long-term care, hospital, home care, or PACE setting.
• Completion of a basic nursing assistant training program.
• Must be medically cleared for communicable diseases.

Responsabilités

• Provide personal care up to 2-person assist.
• 1:1 interaction in DHC and CL as outlined in care plan.
• Assist with activities, meals, toileting, and nutritional needs.
• Grocery shopping, laundry, homemaking, and coordinating care.
• Document care timely and report deviations to supervisor.
