Job summary

PACE Southeast Michigan is seeking a Community Living Associate to support residents in the community living setting, transport, and day health center activities under supervision. You will perform tasks outlined in each participant’s care plan, provide compassionate care, and help residents achieve greater independence.

The role requires a high school diploma or GED, CNA training, CPR, and a valid Michigan driver’s license.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• CNA training and certification preferred; at least 1 year with frail elderly.
• Experience in long-term care, hospital, home care, or PACE setting.
• Completion of a basic nursing assistant training program.
• Certificate of completion from a formal training program outside PACE SEMI.
• Participation in annual in-service training (infection control, TB, safety, BLS).
• Ability to build interpersonal relationships and work across departments.
• Ability to work with diverse ethnic and cultural backgrounds.

Responsibilities

• Provide care and services per the participant's care plan.
• Assist with activities of daily living and mobility as outlined.
• Assist with meals, toileting, and incontinence needs.
• Coordinate care with participant and DHC per plan.
• Document accurately and timely; report deviations or complaints.
• Respond to emergencies per established policies; work within scope.
