Job summary

PACE Southeast Michigan seeks a Community Living Associate (CLA) to provide supportive services to residents in community settings, during transport, and at day health centers. You will follow the participant care plan, assist with activities of daily living, meals, mobility, and coordinate with the clinical team.

Qualifications

• HS diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• Certification is optional though preferred, but must have one year experience with a frail or elderly population.
• Experience must be in a long-term care, hospital, home care setting, or PACE setting.
• Mandatory basic nursing assistant training program completion.

Responsibilities

• Provide care or services per the participant's care plan.
• Assist with activities of daily living and mobility.
• Assist with meals, toileting, and hygiene needs.
• Coordinate care with the DHC and clinical team.
• Document accurately and timely.
