Job summary

The Alden Network is seeking an experienced Resident Care Coordinator to ensure up-to-date care plans and MDS for residents, maintaining documentation aligned with federal, state, and local standards.

This role requires an active RN or LPN license in Illinois, CPR certification, and strong leadership with the ability to work with residents, families, and staff. Proficiency with electronic records is expected.

Qualifications

• Current, unencumbered license to practice as a RN, or LPN in this state.
• Must possess current CPR certificate or become certified within 90 days of employment.
• Working knowledge of OBRA, IDPH and IDPA standards and methods of documenting according to those standards.
• Must be able to read, write and speak the English language in an understandable manner.
• Must possess the ability to deal tactfully with residents, family members, visitors, personnel, government agencies, and the general public.
• Must possess leadership ability and willingness to work harmoniously with personnel.

Responsibilities

• Review the department’s policy and procedures for revisions and make recommendations to the Administrator.
• Oversees the scheduling, completion, and accuracy of all MDS item sets.
• Coordinate the development of a written plan of care for each resident with measurable goals.
• Chairs resident care plan conference and relays concerns to the Administrator.
• Oversee that residents and families are contacted for participation in care plan conferences per RAI guidelines.
• Plan, conduct, and participate in In-service Education Programs related to RAI process.
• Assist in implementing and maintaining an orientation program for new employees to the RAI process.
• Attend relevant meetings as directed by the Administrator.
• Monitor resident medical records for consistency and content; address discrepancies with the appropriate department manager.
