Job summary

The Alden Network is seeking a Resident Care Coordinator in Illinois. You will ensure an up-to-date care plan and MDS for every resident, aligning documentation with federal, state, and local standards while following facility policies under the Administrator’s direction.

You will lead care plan conferences, coordinate multidisciplinary approaches, and monitor records for consistency, accuracy, and compliance with the RAI process.

Qualifications

• Active RN or LPN license in Illinois.
• CPR certification current or obtained within 90 days of employment.
• Knowledge of OBRA, IDPH/IDPA (IL) and Wisconsin DQA (WI) standards for documentation.
• Ability to read, write, and speak English clearly.
• Ability to interact tactfully with residents, families, visitors, staff and the public.
• Proven leadership and teamwork abilities.

Responsibilities

• Review department policies and procedures and update Administrator on revisions.
• Oversee scheduling, completion, and accuracy of all MDS item sets.
• Develop written care plans for each resident with problems, goals and multidisciplinary approaches.
• Chair resident care plan conferences and relay concerns to the Administrator.
• Ensure residents and families are contacted for care plan conferences per RAI guidelines.
• Plan and participate in In-service Education related to the RAI process.
• Assist in orienting new staff to the RAI process and data entry.
• Monitor medical records for consistency and compliance; address discrepancies with supervisor.
