Job summary

Worland Health and Rehabilitation is seeking a Resident Care Manager (RCM) to oversee the delivery of total care to residents. You will provide general nursing care and supervise Licensed Nurses and CNAs within the facility.

Responsibilities include medication administration, observation and reporting of patient changes, vital signs, and ensuring compliance with applicable regulations and internal policies. This role emphasizes geriatric care and rehabilitation within a long-term care setting.

Qualifications

• Graduate of an accredited nursing program.
• Current LPN or RN licensure in the state of employment.
• Demonstrated knowledge of nursing process, especially for geriatric residents in long-term care and rehabilitation.

Responsibilities

• Oversee administration of total resident care and supervise licensed nurses and CNAs.
• Administer prescribed medications and treatments per nursing standards.
• Monitor residents, record significant changes, and notify supervisor, family, and physician as needed.
• Document vital signs and respond to deviations in resident condition.
• Enforce nursing staff compliance with state, federal regulations and facility policies.

Job Summary:

Worland Health and Rehabilitation is currently looking for a Resident Care Manager (RCM) to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
• Takes vital signs to detect deviations from normal and assess condition of resident.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience:

• Required: Graduate of an accredited school of nursing.
• Required: Current LPN or RN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
