Evergreen Healthcare Group’s Rawlins Rehabilitation and Wellness in Rawlins, WY seeks a Resident Care Manager (RCM) to oversee total care for residents and supervise Licensed Nurses and CNAs. The role includes administering medications, monitoring conditions, and coordinating care plans in a geriatric setting.

Qualified candidates hold an active RN or LPN license in Wyoming and have nursing process knowledge.

Qualifications

• Graduate of an accredited school of nursing.
• Current LPN or RN licensure in the state of employment (Wyoming).
• Knowledge of nursing process in geriatric, long-term care and rehabilitation.

Responsibilities

• Obtains and administers prescribed medications and treatments per nursing techniques.
• Observes residents, records significant conditions and reactions, and notifies supervisor/physician.
• Takes vital signs to detect deviations from normal and assess condition.
• Oversees disciplinary action for nursing staff non-compliance with regulations and policies.
• Modifies resident care plans and ADL/Aide flow sheets as conditions change.

Job Summary

Rawlins Rehabilitation and Wellness is currently looking for a Resident Care Manager (RCM) to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
• Takes vital signs to detect deviations from normal and assess condition of resident.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience

• Required: Graduate of an accredited school of nursing.
• Required: Current LPN or RN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
