Job summary

Midwest Health, Inc. seeks a Resident Care Coordinator to manage and direct resident services in our community. The role reports to the Executive Director and requires oversight of policy adherence, safety, and quality of care.

You will supervise staff, coordinate with physicians and pharmacies, and ensure that state and federal regulations are followed. Strong communication with residents’ families is essential for care planning.

Qualifications

• Must have a valid professional license (LPN or RN) and be in good standing with the state.
• Must have a valid CPR certification.
• Must have basic computer skills.
• Must be able to read, write, speak, and understand the English language.
• Must be able to work beyond normal hours and on weekends/holidays when necessary.
• Must be able to assist in the evacuation of residents during emergencies.

Responsibilities

• Effectively manage, direct and administer resident services.
• Direct and supervise staff and coordinate with physicians and pharmacy as required.
• Ensure compliance with company policies, procedures and state/federal regulations.
• Maintain confidentiality of resident and family information.
• Monitor adherence to nursing care policies and participate in staff selection and training.
• Engage with families to communicate care plans and service agreements.
