Job summary

Cogir Senior Living seeks a Resident Care Coordinator in Albany, OR to bridge residents, families, and the care team. You will schedule shifts, supervise staff, and ensure quality assisted living services across care areas, with emphasis on medication administration and direct resident care as needed.

You will collaborate with home health agencies, coordinate admissions with the sales team, maintain accurate records, and uphold state regulations. Weekend rotation may be required.

Qualifications

• High School Diploma or equivalent.
• Valid state medication administration license (CNA/CMA/CMT/QMAP) or meet state requirements for medication administration in assisted living.
• LPN/LVN nursing license is a plus.
• Current First Aid and CPR license or ability to obtain.

Responsibilities

• Schedule caregiving and medication technician shifts to ensure adequate coverage and staffing guidelines.
• Participate in recruitment and interview process for new care team members.
• Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
• Supervise the medication room and orders, collaborating with medication technicians and caregivers.
• Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
• Coordinate care planning with home health agencies on-site, within community policies.
• Coordinate admissions with the sales team, including supervising move-ins.
• Complete family/resident admission interviews.
• Facilitate effective communication among residents, families, and healthcare providers to ensure seamless care delivery.
• Address residents' special needs, requests, and complaints.
• Assist with medication administration and provide direct care to residents as needed.
• Understand the community's care regulations, as well as state-specific regulations.

Education

High School Diploma or equivalent

Medication administration license (CNA/CMA/CMT/QMAP) or state requirements
