Job summary

Cogir Senior Living in Scottsdale, AZ seeks a Resident Care Coordinator to bridge residents, families, and the care team. You will assist in scheduling, supervise staff, and ensure quality assisted living services while collaborating with health providers on-site.

You will help with medication administration, handle admissions, and coordinate care planning with home health agencies, maintaining resident records and complying with state regulations.

Qualifications

• High School diploma or equivalent required.
• Valid state medication administration license or meet state requirements.
• LPN/LVN license is a plus.
• Current First Aid and CPR certification preferred.

Responsibilities

• Schedule and assign caregiving and med tech shifts for coverage.
• Participate in recruitment and interview process.
• Supervise, train, and support care team members.
• Oversee medication room and orders with care team.
• Maintain accurate resident records with care plans and docs.
• Coordinate care planning with on-site home health agencies.
• Coordinate admissions with sales, supervise move-ins.
• Conduct family/resident admission interviews.
• Facilitate communication among residents, families, providers.
• Address special needs, requests, and complaints.
• Assist with medication administration and direct resident care as needed.
• Understand state and community care regulations.
