Job summary

Burlington Health and Rehab is seeking a compassionate Resident Care Coordinator to support residents and their families from admission to discharge. This full-time on-site role focuses on welcoming residents, coordinating admissions and discharges, and ensuring a positive experience.

You will work with Admissions, Marketing, and Social Services to address resident needs, conduct rounds, and maintain welcoming common areas. Strong communication and organizational skills are essential.

Qualifications

• High school diploma or equivalent required; associate degree preferred.
• Experience in healthcare, hospitality, or customer service preferred.
• Strong communication and interpersonal skills.
• Highly organized with attention to detail.
• Ability to multitask in a fast-paced environment.
• Compassionate, patient, and resident-focused attitude.

Responsibilities

• Ensure all open rooms are clean, staged, and move-in ready.
• Coordinate admissions and discharges with Admissions, Marketing, and Social Services.
• Conduct rounds to address resident needs and requests.
• Maintain clean, organized, and welcoming common areas.
• Communicate effectively with residents and families, providing information or coordinating follow-ups.
• Conduct follow-up calls with families (Caring Partner calls).
• Confirm required equipment (oxygen, wheelchair, etc.) is ready prior to arrival.
• Greet new residents and families upon admission.
• Assist with admission documentation with families.
• Coordinate discharge readiness and timing.
• Conduct a resident satisfaction survey.
• Coordinate post-discharge room turnover with housekeeping and admissions.
