Job summary

Burlington Health and Rehabilitation Center is seeking a compassionate Resident Care Coordinator to enhance the resident and family experience from admission to discharge.

The role requires strong organization, clear communication, and coordination with Admissions, Marketing, and Social Services. A high school diploma is required; healthcare-related experience is preferred and encouraged.

Qualifications

• High school diploma or equivalent required; associate degree preferred.
• Experience in healthcare, hospitality, or customer service preferred.
• Strong communication and interpersonal skills.
• Highly organized with attention to detail.
• Ability to multitask in a fast-paced environment.
• Compassionate, patient, and resident-focused attitude.
• Previous experience in healthcare helpful, but not necessary.
• Ability to work under stress and in emergency situations.

Responsibilities

• Ensure all open rooms are clean, staged, and move-in ready.
• Coordinate with Admissions, Marketing, and Social Services for admissions and discharges.
• Conduct routine rounds to address resident needs and requests.
• Maintain clean, organized, and welcoming common areas.
• Communicate effectively with residents and families, providing information or coordinating follow-ups.
• Conduct follow-up calls with families "Caring Partner" calls.
• Confirm required equipment (oxygen, wheelchair, etc.) is ready prior to arrival.
• Greet new residents and families upon admission.
• Assist with inventory of belongings, clothing labeling, and orientation to services and amenities.
• Help complete admission documentation with families.
• Coordinate discharge readiness and timing.
• Conduct a brief resident satisfaction survey.
• Coordinate post-discharge room turnover with housekeeping and admissions.
