The role collaborates with hospitals, physicians, social workers, discharge planners, families, insurance companies, and our clinical team to ensure each admission is handled efficiently and professionally. This full-time position supports census and occupancy goals and requires excellent communication.

Qualifications

• Experience in skilled nursing, long-term care, rehabilitation, or hospital settings.
• Admissions, intake, case management, or healthcare coordination preferred.
• Strong communication and attention to detail.

Responsibilities
