StoryPoint Forest Hills in Grand Rapids, MI seeks a dedicated Care Specialist to provide and coordinate resident care and lead the care team. You will follow service plans and participate in orientation and training.

Responsibilities include administering medications per orders, maintaining infection control, documenting daily charts, and promoting a positive, collaborative environment for residents, families, and staff.

Formación

• High school diploma or GED with Care Specialist I/II within 90 days of hire.
• At least 6 months of care specialist or equivalent experience.
• CPR/First Aid certification (or obtain within 30 days of hire).
• Proficient in Microsoft Office in a Windows environment.

Responsabilidades

• Provide and coordinate resident care to ensure quality care.
• Follow resident service plans and update as required.
• Administer medications per physician orders and policies.
• Review physician orders with eMAR and pharmacy support.
• Perform daily charting, documentation, and reporting.
