Recover-Care Heartland in Shawnee, KS is seeking a Resident Care Coordinator to assist the Director of Nursing and RN Assessment Coordinator in ensuring that documentation aligns with Federal, State, and Certification guidelines and to coordinate the RAI process.

The role includes overseeing MDS, CAAs, and Interdisciplinary Care Plans, performing audits, and using PointClickCare for forms. Daily duties include completing assessments and supporting IDT meetings and care plan updates.

Qualifications

• Must hold an active RN or LPN license in the practicing state.
• Strong knowledge of MDS/RAI processes and OBRA regulations.
• Experience with regulatory surveys and documentation standards.

Responsibilities

• Ensure documentation meets Federal and State guidelines.
• Coordinate the RAI process for MDS, CAAs, and IDT care plans.
• Monitor PCC for admissions paperwork and referrals.
• Complete patient assessments and data collection as required.
• Support audits and regulatory surveys with accurate records.
• Update care plans and documentation timelines for new admissions.
