Job summary

Diversicare is seeking a Care Assessment Coordinator to promote resident-centered care and ensure accuracy and compliance in MDS assessments. This role leads the center’s case management, discharge planning, and care coordination efforts, partnering with the interdisciplinary team to deliver timely, compliant documentation and optimal outcomes.

The ideal candidate holds an RN license, has MDS/RAI experience, and understands PDPM and managed care processes.

Qualifications

• Must hold a current RN license in state of employment.
• LPNs will be considered in centers where an RN is present in the department.
• Previous MDS/RAI experience.
• Knowledge of PDPM, managed care reimbursement, utilization and case management as well as discharge planning preferred.
• Experience providing strategic leadership, managing teams and leading projects.

Responsibilities

• Maintain and improve compliance with state and federal MDS requirements by delivering accurate, timely assessments and care plans, while promoting a culture of collaboration, quality, and resident-centered care.
• Coordinates the RAI process, ensures accurate MDS assessments, collaborates with the team to ensure thorough documentation, and maintains MDS schedules.
• Monitors Quality Measures and collaborates with the interdisciplinary team to ensure MDS accuracy.
• Ensures Medicare and Medicaid regulatory guidelines are completed accurately and timely.
• Determines need for non-coverage notification and coordinates communication with patients/families/payers and the interdisciplinary team.
• Facilitates daily care coordination meetings, End of Month Billing Review Foundational Process, and discharge planning.
