Recover-Care Heartland in Kansas is seeking a Resident Care Coordinator (LPN/RN) to support the Director of Nursing and ensure documentation complies with Federal, State, and Certification guidelines. You will coordinate the RAI process, manage MDS, CAAs, and the interdisciplinary care plan, and work with the IDT to update care plans, attend meetings, and drive quality documentation across the facility.

Qualifications

• Graduate of an approved RN or LPN program and licensed in the state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility.
• Solid knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement; OBRA knowledge.

Responsibilities

• Ensure documentation meets Federal, State, and Certification guidelines.
• Coordinate the RAI process and MDS to ensure timely completion of care plans.
• Monitor PCC system for RAI tracking, discharge/reentry, and Medicaid forms.
• Review point-of-care documentation and update as needed.
• Develop new admission baseline care plans within 48 hours.
• Complete PCC care plan reviews and restorative nursing updates.
• Communicate with IDT and attend key meetings.
