Job summary

Diversicare invites experienced clinicians to join our team as Care Assessment Coordinator. You will oversee MDS/RAI accuracy and regulatory compliance, coordinating comprehensive care plans with an emphasis on resident-centered care and collaboration across disciplines.

The role emphasizes leadership, documentation accuracy, and timely communications with residents, families, and external stakeholders to support optimal outcomes and seamless discharge planning.

Qualifications

• Must hold a current RN license in state of employment.
• LPNs considered in centers where an RN is present in the department.
• MDS/RAI experience preferred.
• Knowledge of PDPM and case management is preferred.
• Strong verbal, written and interpersonal skills are required.

Responsibilities

• Coordinate and lead the MDS/RAI process to ensure accurate assessments.
• Monitor regulatory compliance for state and federal requirements.
• Oversee care planning and discharge planning processes with the interdisciplinary team.
• Facilitate communication with residents, families, and payers.
• Support documentation, audits, and timely electronic submissions of required reports.
• Lead care coordination meetings and provide education on care management processes.
• Manage coordination of restorative nursing and post-discharge communication.
