Job summary

Merriam Gardens Healthcare and Rehab Center in Shawnee, KS, is seeking a Resident Care Coordinator to assist the Director of Nursing and the RN Assessment Coordinator. You will ensure that documentation complies with federal, state, and certification guidelines.

You will coordinate the RAI process, ensure timely MDS, CAAs, and Interdisciplinary Care Plans, and monitor PCC in daily operations with the IDT. Qualified candidates hold an RN or LPN license, with SNF experience preferred.

Qualifications

• Must hold an active RN or LPN license in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Assist Director of Nursing and RN Assessment Coordinator to ensure documentation meets Federal/State/Certification guidelines.
• Coordinate the RAI process including timely and complete MDS, CAAs, and Interdisciplinary Care Plan.
• Monitor PCC documentation, audits, and ensure compliance with standards.
• Attend and participate in facility-level meetings to update care plans and records.
• Complete patient assessments and communications with the IDT to support resident care.
