Royal Crown Homes LLC is seeking a dedicated Designated Coordinator/Manager in Bloomington, MN to lead the coordination of services for residents, ensuring compliance with state regulations and a high quality of life. This role demands compassionate leadership and strong organizational skills to guide DSPs and DCs.

Qualifications

• Experience in residential or community settings serving individuals with disabilities or mental health needs.
• Knowledge of Minnesota 245D licensing rules and DHS compliance requirements.
• Supervisory or management experience preferred.
• Strong organizational, communication, and documentation skills.
• Ability to pass a DHS background study.
• Valid driver's license and reliable transportation.
• Designated Coordinator qualifications under Minnesota Statute 245D.
• Compassionate, solution-oriented and person-centered care.

Responsibilities

• Develop, implement, and monitor ISPs for residents in compliance with DHS guidelines.
• Serve as the primary contact for residents, families, case managers, and county reps.
• Supervise, train, and support DSPs and other DCs to ensure quality care.
• Conduct assessments and document progress to adjust care plans.
• Ensure compliance with 245D licensing standards and state/federal regulations.
• Coordinate medical appointments, community activities, and daily staff scheduling.
• Manage incident reporting, investigations, and corrective actions when needed.
• Foster a safe, respectful, and person-centered environment for all residents.
