Job summary

Cogir Senior Living in Tacoma, WA seeks a dedicated Resident Care Coordinator to act as liaison between residents, families, and the care team, supporting the Health and Wellness Director. You will assist scheduling, supervise staff, and ensure quality assisted living services across resident care areas.

Responsibilities include supervising shifts, coordinating care with teams and external providers, and providing direct resident care when needed while ensuring compliance with state regulations

Qualifications

• High School Diploma or equivalent.
• Must have a valid state medication administration license (CNA/HCA) and meet state requirements.
• Nurse Delegation and Diabetes Specialty Certificates, or willingness to obtain within a prescribed period.
• An LPN nursing license is a plus.
• Current First Aid and CPR license or ability to obtain.

Responsibilities

• Schedule and assign caregiving and medication tech shifts to ensure adequate coverage.
• Participate in recruitment and interview process for new care team members.
• Supervise, train, and support care team members and promote teamwork.
• Supervise the medication room and orders and collaborate with caregivers.
• Maintain resident records, care plans, incident reports, and medical documentation.
• Coordinate care planning with home health agencies on-site and with policies.
• Coordinate admissions with the sales team, including move-ins.
• Conduct family/resident admission interviews.
• Facilitate communication among residents, families, and healthcare providers.
• Address residents’ special needs, requests, and complaints.
• Assist with medication administration and provide direct care as needed.
• Understand care regulations and state-specific regulations.
