Job summary

Merrill Gardens at Cedar Mill is seeking a Resident Care Coordinator to oversee coordinated resident care services and support personalized care planning for assisted living residents. You will help with move-ins, assessments, and transitions of care, ensuring smooth, compassionate service.

The role requires 3+ years in a related healthcare setting, knowledge of care planning, documentation, and regulations, plus strong communication and leadership abilities to support staff and families.

Qualifications

• High school diploma or GED required; CNA/HCA credentials preferred or required per state regulations.
• 3+ years of experience in assisted living, senior living, or related healthcare.
• Knowledge of resident care planning, documentation, medication management, and regulations.
• Experience supporting caregiver training, staffing, scheduling, or team leadership.
• Strong communication, customer service, organizational, and problem-solving skills.
• Commitment to providing exceptional resident-centered care and positive relationships.

Responsibilities

• Coordinating resident care services and supporting personalized service planning for assisted living residents.
• Assisting with resident move-ins, assessments, care planning, and transitions of care.
• Monitoring resident health, functional and psychosocial needs and reporting changes.
• Ensuring resident records, service plans, medication documentation, and caregiver charting are accurate and compliant.
• Coordinating caregiver orientation, training, and ongoing education; supporting team development.
• Assisting with staffing, scheduling, and shift coverage to meet resident care needs.
• Supporting quality assurance, regulatory compliance, and state survey preparedness.
• Providing hands-on resident care support and liaising with residents, families, physicians, and leadership.

Our Resident Care Coordinators are responsible for:

• Coordinating resident care services and supporting personalized service planning for assisted living residents.
• Assisting with resident move-ins, assessments, care planning, and transitions of care.

• Ensuring resident records, service plans, medication documentation, and caregiver charting are accurate and compliant with company and regulatory standards.
• Coordinating caregiver orientation, training, and ongoing education while supporting team development and performance.
• Assisting with staffing, scheduling, and shift coverage to ensure resident care needs are met.
• Supporting quality assurance initiatives, regulatory compliance efforts, and state survey preparedness.
• Providing hands‑on resident care support and serving as a liaison between residents, families, physicians, and community leadership.

If you are someone with:

• A high school diploma or GED required; active CNA/HCA credentials preferred or required in accordance with state regulations.
• 3+ years of experience in assisted living, senior living, or a related healthcare environment.
• Knowledge of resident care planning, documentation, medication management, and assisted living regulations.
• Experience supporting caregiver training, staffing, scheduling, or team leadership.
• Strong communication, customer service, organizational, and problem‑solving skills.
• A commitment to providing exceptional resident‑centered care while fostering positive relationships with residents, families, and team members.

Physical Requirements:

This role involves frequent standing, walking, lifting (up to 75 lbs), and assisting with resident care. Regular bending, reaching, and physical movement are required throughout the workday.
