Job summary

Merrill Gardens in Portland, OR is seeking a Memory Care Coordinator at Cedar Mill to support memory care residents through coordinated personal care, move-ins, assessments and care planning.

This on-site role emphasizes accurate documentation, regulatory compliance, and collaboration with families and staff to ensure compassionate, high-quality outcomes for residents and their loved ones.

Qualifications

• High school diploma or GED required; college degree preferred.
• 2+ years of experience in senior living, especially memory care or assisted living.
• Excellent communication with residents, families, and staff.
• Strong documentation and regulatory compliance skills.

Responsibilities

• Supporting memory care residents through coordination and delivery of personal care services
• Assisting with resident move-ins, assessments, and care planning
• Monitoring and documenting resident conditions and service needs
• Ensuring accurate records and regulatory compliance
• Providing hands-on personal care and engaging with families and staff to support quality outcomes
• Acting as a key liaison between residents, families, and department leaders

Physical Requirements:

This role involves frequent standing, walking, lifting (up to 75 lbs), and assisting with resident care. Regular bending, reaching, and physical movement are required throughout the workday.
