Merrill Gardens at Cedar Mill is seeking a Resident Care Coordinator to oversee personalized care plans, assist with move-ins and transitions, and monitor resident health. Ideal candidates have CNA/HCA credentials and 3+ years in assisted living, with excellent communication and organizational skills.

Qualifications

• High school diploma or GED required; active CNA/HCA credentials preferred or required per state regulations.
• 3+ years of experience in assisted living, senior living, or related healthcare.
• Knowledge of resident care planning, documentation, medication management, and regulations for assisted living.
• Experience supporting caregiver training, staffing, scheduling, or team leadership.
• Strong communication, customer service, organizational, and problem-solving skills.
• Commitment to providing exceptional resident-centered care and fostering positive relationships.

Responsibilities

• Coordinating resident care services and supporting personalized service planning for residents.
• Assisting with resident move-ins, assessments, care planning, and transitions of care.
• Monitoring resident health and reporting significant changes to appropriate parties.
• Ensuring resident records, service plans, medication documentation, and caregiver charting are accurate and compliant with standards.
• Coordinating caregiver orientation, training, and ongoing education while supporting team development.
• Assisting with staffing, scheduling, and shift coverage to meet resident care needs.
• Supporting quality assurance and regulatory compliance efforts.
• Providing hands-on resident care support and acting as liaison between residents, families, physicians, and leadership.
