Job summary

Certus Healthcare Management seeks a compassionate Case Manager to coordinate care for residents across our Cleveland, OH area facilities. You will develop individualized care plans, conduct assessments, and collaborate with providers, families, and staff to ensure comprehensive support.

This on-site role offers a Monday–Friday, 9am–5pm schedule, opportunities for crisis intervention, detailed documentation, and engagement with a multidisciplinary team in a family-centered setting.

Qualifications

• Assess patient needs, develop individualized care plans, coordinate services across disciplines.
• Strong communication and interpersonal skills for working with patients, families, and a diverse clinical team.
• Organizational and time management skills with detailed documentation and caseload management.
• Knowledge of healthcare regulations and utilization review in case management.
• Proficiency with electronic health records and standard office software.
• Relevant clinical or human services experience in hospital, long-term care, or community health.
• Associate or Bachelor’s degree in Nursing, Social Work, or related field; licensure or CCM preferred.
• On-site work in Cleveland, OH within an interdisciplinary environment.

Responsibilities

• Develop and implement individualized care plans for residents.
• Conduct assessments to evaluate health status and needs for services.
• Coordinate with healthcare providers, families, and community resources to ensure comprehensive support.
• Provide crisis intervention and support to residents facing challenges or emergencies.
• Facilitate communication between residents, families, and staff to promote understanding and collaboration.
• Monitor progress and adjust care plans as necessary to achieve optimal outcomes.
