Job summary
Certus Healthcare Management is seeking a compassionate Case Manager to join our team in Mayfield Heights, OH. You will coordinate care for residents, develop individualized plans, and ensure services meet each resident's needs. The role supports families, coordinates with providers, and monitors progress to improve quality of life.
Typical hours are 9am–5pm, Monday–Friday, with some flexibility; salary discussed at interview. The corporate office is in Mayfield Heights, Ohio.
Qualifications
- Experience in case management, medical authorization, or healthcare billing.
- Strong Medicare, Medicaid, and Managed Care payor rules knowledge.
- Excellent communication, organization and follow-up skills.
- Ability to work independently, multi-task and meet deadlines.
- LPN/SWA or similar clinical background preferred but not required.
- Experience with EMR systems and payor authorization portals.
- Knowledge of Ohio Medicaid processes.
Responsibilities
- Develop and implement individualized care plans for residents.
- Conduct assessments to evaluate health status and service needs.
- Coordinate with providers, families and community resources.
- Provide crisis intervention when needed.
- Facilitate communication between residents, families, and staff.
- Monitor progress and adjust care plans to achieve outcomes.
