We are seeking a compassionate and dedicated Case Manager to join our team at Certus Healthcare Management. In this role, you will play a vital part in coordinating care for our residents, ensuring they receive personalized support tailored to their unique needs. Your contributions will directly impact the quality of life for those we serve.
This position works out of our corporate office located in Mayfield Heights, Ohio. The typical work schedule for this position is Monday through Friday from 9am to 5pm. Start and end times have some flexibility and can be discussed during interview process.

Experience working in case management, medical authorization processes, or healthcare billing.
Strong understanding of Medicare, Medicaid, and Managed Care payor rules.
Excellent communication, organization, and follow-up skills.
Ability to work independently, multi-task, and meet deadlines.
Prior experience in skilled nursing, long-term care, or hospital case managementpreferred.
Licensed Practical Nurse (LPN), Social Work Assistant (SWA), or similar clinical background preferred but notrequired.
Experience using EMR systems and payor authorization portals.
Knowledge of Ohio Medicaid processes
