Job summary

PACS Group is seeking a dedicated nursing aide to provide routine daily nursing care for assigned residents in accordance with their care plans. You will identify residents, assist with daily activities, and ensure a warm, safe environment while documenting observations for the care team.

Responsibilities include reporting changes to the Nurse Supervisor, cooperating with staff, and supporting residents during meals, transfers, and procedures as directed.

• Provide routine daily nursing care to residents according to assessment and care plan.
• Identify residents before treatments or meals using wristbands or photo IDs.
• Document care and observations clearly and promptly.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse.
• Escort residents, assist with daily activities, and maintain a warm, safe environment.
