Job summary

PACS Group in California seeks a dedicated nursing aide to provide routine daily care to residents in accordance with each resident's care plan. You will identify residents, assist with meals and daily activities, and ensure a warm, safe environment on your shift.

You will report changes in condition to the Nurse Supervisor, document entries accurately, and support with transfers, transportation, and basic treatments as trained.

Qualifications

• Must provide routine daily nursing care per resident assessment and care plan.
• Identify residents before procedures using wristband/photo.
• Maintain accurate flow sheets and charts with clear entries.
• Cooperate with nurses and supervisors to ensure quality care.

Responsibilities

• Identify residents and escort to rooms, ensuring comfort and safety.
• Assist with daily activities, meals, and personal care as trained.
• Record vital signs and care notes in the resident’s chart.
• Report changes in residents' condition promptly to the Nurse Supervisor/Charge Nurse.
• Follow policies on exposure to blood/body fluids and infection control.
• Support residents during transfers and transportation between rooms or areas.
