Job summary

PACS is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care to residents in our post-acute facility. You will follow care plans, assist with hygiene, mobility, meals, and comfort under supervision.

Responsibilities include documenting care, monitoring residents, reporting changes promptly, and maintaining a safe, respectful, and warm environment for all residents. A state CNA license and CPR training are preferred.

Qualifications

• Must hold a state Certified Nursing Assistant license.
• Ability to read, analyze, and interpret information.
• CPR training preferred.

Responsibilities

• Identify residents before care is provided using wristband/photo card.
• Record entries accurately on flow sheets and charts.
• Report changes in resident condition promptly to Nurse Supervisor/Charge Nurse.
• Assist residents with daily activities: hygiene, meals, mobility, and comfort.
• Maintain resident room readiness and safety.
• Protect resident privacy and confidential information.
