Job summary

Qualifications

• Nursing assistant or equivalent training preferred.
• Ability to follow care plans and report changes.
• Strong communication and observation skills.

Responsibilities

• Identify residents before administering treatments using wristband or photo file.
• Record entries clearly and legibly on flow sheets and charts.
• Report changes in residents' condition to Nurse Supervisor promptly.
• Ensure rooms are prepared for new residents and assist with transfers.
• Greet residents, introduce to roommates and staff, and provide comfort.
• Maintain privacy and protect residents’ health information.
• Check residents routinely to meet personal care needs.
• Assist with daily activities and meals as assigned.
• Observe skin integrity and report signs of pressure or ulcers.
