Job summary

Westminster Baldwin Park is seeking a Social Services/Admissions Coordinator to support the Administration/Health Center. The role covers social services and admissions for a 20-bed rapid recovery unit, ensuring resident needs are met and occupancy is maintained.

You will coordinate admissions, liaise with families and physicians, manage grievances, QA, and compliance with Medicare/Medicaid requirements while fostering compassionate, person-centered care.

Qualifications

• Bachelor’s Degree in Social Work or Sociology or health-related field, or RN with 2+ years experience.
• Three years’ experience in a skilled nursing facility or related area is required.
• One year in long-term care with Medicare/Medicaid knowledge; post-acute rehab experience preferred.
• Knowledge of admission documentation and process.

Responsibilities

• Prepare psychosocial assessments and care plans for residents.
• Assist during Admission by tours and collecting signatures for documentation.
• Resident/family outreach; advocate for residents and primary family contact.
• Coordinate access to community resources and healthcare providers.
• Manage discharge planning and appropriate discharges to less restrictive settings.
• Resolve grievances and conduct investigations; conduct staff in-service training.
• Complete QA/Audits and monitor psychotropic meds and psychoactive binder.
