Job summary

Westminster Baldwin Park is seeking a Memory Support Coordinator to lead the Memory Support Unit. The role develops, implements, and maintains the Memory Support program, maintains relationships with residents, families, and team members, and ensures budget adherence and regulatory compliance.

The position reports to the Health Services Administrator and is a full-time, salary role. Qualifications include a health-related bachelor’s degree and relevant certifications; experience in long-term

Qualifications

• Bachelor’s degree in a health-related field is required or preferred.
• NHA, BSW, or Recreation Therapy degree are favored.
• ALF Core training and certification are preferred.
• CPR and First Aid Certification are required.

Responsibilities

• Supervises staff with interviewing, training, and performance management.
• Plan and operate department to meet budget goals (occupancy, staffing).
• Coordinate admissions into Memory Support and communicate with stakeholders.
• Develop and maintain individualized service plans and Memory Support elements.
• Ensure memory care services align with evidence-based methods and company standards.
• Oversee clinical aspects of care and medication handling.
