Job summary

Westminster Communities of Florida seeks a Director of Social Services / Admissions Coordinator to handle psychosocial assessments, admissions coordination, and discharge planning for a 20-bed rapid recovery unit and general population. Work in the Administration/Health Center under the Health Services Administrator.

The role requires a Social Work degree (or RN with relevant experience) and Medicare/Medicaid knowledge, with strong communication and advocacy for residents and families.

Qualifications

• Bachelor’s Degree in Social Work or Sociology or health-related field; RN with 2 years experience accepted.
• 3 years’ experience in skilled nursing facility or medically related area is required.
• One year in long term care with Medicare/Medicaid knowledge preferred.

Responsibilities

• Prepare psychosocial assessments and care plans for residents.
• During admissions, tour facility with prospective residents and families.
• Serve as liaison to families, representatives, and physicians.
• Manage discharge planning and appropriate discharges to less restrictive settings.
• Resolve resident/family grievances and conduct investigations as needed.
• Oversee QA/Audits and monitor psychotropic medications.
